Davis County Birth and Death Records

The Davis County Health Department issues birth and death certificates. Interested parties in Davis County may get birth certificates from 1926 until the present from this agency. However, only incidents reported prior to August 1, 2006, are eligible for death certificates.

Requesters may get a birth certificate by submitting a completed Birth Certificate Request Form.
